چکیده
Background Rheumatoid arthritis (RA) is one of the most common diseases worldwide with a need for rehabilitation despite recent advantages in pharmacological treatment options. Furthermore, data show numerical increase years life disability RA patients Germany from 1990 to 2017. Limitations work participation (WP) also remain highly prevalent. Objectives We investigate development different domains WP (unimpaired WP, presenteeism, absenteeism, and no employment) at patient level under drug therapy over 12 months understand impact treatment. Methods The multicenter ERFASS study prospectively enrolled seropositive an active flare disease initiating treat-to-target (T2T) [1]. Patients were between 01/2018 12/2019. examine working-age (18-65 years) excluding students old-age pensioners using WPAI complete dataset all three measurement time points (month 0, 6 12). Disease activity was measured DAS28. Results A total 122 (73.0% female) mean age 52.8 (SD: 8.5) included. according DAS28-CRP decreased significantly course 4.2 baseline 2.5 month (p<.001). proportion non-working increased numerically 30.3% 36.1% (p=0.085). absenteeism 22.1% 12.3% (p=0.033). There small changes presenteeism 12: 35.2%; p=0.086). unrestricted 10.7% 16.4% (p<0.001) (Table 1). Graph 1 shows quantitatively relevant sankey diagram. group not working mainly gains growth absenteeism. unimpaired proportionally become more because or improve their WP. However, we detected substantial initially facing impairments as likely during T2T. At measure see fluctuations Conclusion Besides good response terms activity, demonstrate that majority unemployed acute does find way back into force within twelve T2T pharmaceutical greatest improvements observed baseline. Although without limitations significantly, it remained low end study. Our provide understanding after which helps tailor occupational concepts. Reference [1]Hoeper, J. R., et al. (2021). A Case of Acute Myeloid Leukemia Caused by Low Dose Methotrexate Used to Treat a Rheumatoid Arthritis Patient
Methotrexate is an anti-rheumatic agent used as a first-line tretment for rheumatoid arthritis. Hematological malignancies like lymphoma or acute myeloid leukemia have been reported to be secondary to treatment with Methotrexate, but are very uncommon. We report here the first Moroccan case of RA patient on low dose MTX, who developed AML. We also reviewed all similar cases.
متن کاملIncorporating the treat-to-target concept in rheumatoid arthritis.
BACKGROUND Recent publications have proposed revisions to disease classification criteria, new definitions of remission, and guidelines for implementing treat-to-target strategies for the management of patients with rheumatoid arthritis (RA). Despite developments leading to this practice-changing approach, the concept of treat to target has not yet been widely accepted or implemented in managed...
متن کاملTreat to target strategy in rheumatoid arthritis: real benefits.
Treating rheumatoid arthritis (RA) with a goal or «Treat to target» strategy is a therapeutic proposal taken from cardiovascular and endocrine literature. It proposes that the therapeutic target in RA should be a state of remission, or an alternative goal could be a low disease activity. Rheumatologists should measure and register disease activity in every clinical visit and if the goal has not...
